Save Code:

You only need a 'Save Code' if you have previously answered part of the survey. If you are responding for the first time, you do not need a 'Save Code'. When you follow the link to take the survey , do not follow the link that reads 'Click here to continue a partially completed form'. Instead, scroll down to the bottom of the screen and click the green button labeled 'Continue'.
