Fill out our online SurveyShare Sales Affiliate, Referral Agent, and Web Site Affiliate registration form.
Print and sign the Sales Affiliate, Referral Agent, and Web Site Affiliate Program Operating Agreement.
Fax or mail the Sales Affiliate, Referral Agent, and Web Site Affiliate Program Operating Agreement to SurveyShare.
Fax or mail the resume or vita of the primary contact person of this application with at least 3 references to SurveyShare.

After your Sales Affiliate application has been approved, your name will appear in SurveyShare's new user sign up page, under the option labeled "How did you find out about SurveyShare?". If a new user selects your name when signing up for SurveyShare, we will mail you a 25% commission check. You will also be able to refer qualified leads to our sales department by filling out this Online Referral Form.
