This is an example Shopping Survey survey. This survey includes questions like:
- Do you do most of your shopping in stores, over the internet, or both equally?
- When shopping for household cleaning items, do you shop mainly at discount stores such as Wal-Mart or K-Mart or do you purchase these items at the grocery store?
- When shopping for clothing do you usually shop at Discount stores, Specialty stores, Malls, Catalogs, or on the Internet?

use this as a starting point to help your own survey instrument. Feel free to
